ABSTRACT

BACKGROUND: Non-adherence and comorbidities are prevalent among hemodialysis patients and are associated with increased mortality and financial burden. We aimed to investigate the influence of major coping strategies (CSs) on non-adherence and comorbidities in hemodialysis patients. METHODS: A total of 49 patients were enrolled. We collected participant data including CS measured by a Korean version of the ways of coping questionnaire (K-WCQ), comorbidities measured by age-adjusted Charlson comorbidity index (CCI), and adherence measured by the 8-item Morisky medication adherence scale (MMAS-8). RESULTS: Regarding major CS, 61.2% of participants reported use of support-seeking CS (SUP group), 14.3% reported use of problem-focused CS (PRO group), and 24.5% reported use of hopeful-thinking CS (HOP group). The mean MMAS-8 score was higher in the PRO group than in the HOP group (P = 0.024). The mean CCI score was lower in the PRO group than in the HOP group (P = 0.017). In the HOP group, the severity of somatic symptoms was positively correlated with the scores for the emotion-focused CS subscale (r = 0.39, P = 0.029) and the hopeful-thinking CS subscale (r = 0.38, P = 0.036) of the K-WCQ. The level of life satisfaction positively correlated with the score for the problem-focused CS subscale in the HOP group (r = 0.40, P = 0.027). CONCLUSION: We should pay more attention to the CSs of hemodialysis patients and provide interventions that promote problem-focused CSs, especially for nonadherent patients with high comorbidity rates who mainly use a hopeful-thinking CS.

ABSTRACT

Bronchopulmonary sequestration (BPS) is a rare congenital malformation of the lower respiratory tract. Most intralobar BPSs are provided with an arterial blood via the thoracic or abdominal aorta but such a supply is rarely found in patients older than 50 years. We report a case of an intralobar BPS with a dual arterial supply from the celiac artery and thoracic aorta in a 50-year-old man presenting with a respiratory tract infection and haemoptysis. To our knowledge, this is the first case report of a BPS supplied by the celiac artery and thoracic aorta in a 50-year-old man.

ABSTRACT

Common causes of acquired tracheoesophageal (T-E) fistula are blunt trauma on the neck or chest, malignancy, long-term mechanical ventilation, and post-intubation injury. Most of the cases are fatal due to severe respiratory infection. We experienced two cases of post-intubation T-E fistula in patients with a history of tracheostomy that developed earlier than usual. One case was caused by excessive cuff pressure and the other by avulsion injury during endotracheal intubation. We can get instructions from these cases that how to prevent T-E fistula because it is hard to treat and causes severe outcomes.

ABSTRACT

We report a case of alopecia areata in a 32-year-old man that developed during the course of cyclosporine therapy following kidney transplantation. Cyclosporine has been reported to be of value in the treatment of alopecia areata. It would therefore appear unlikely that alopecia areata would develop in our patient while he was undergoing cyclosporine therapy. To our knowledge, this is a rare case report of alopecia areata in a patient taking cyclosporine in Korean dermatologic literature.

ABSTRACT

Psoriasis is a relatively common disorder that affects at least 1-2% of the population, however, the pathogenesis is not fully understood yet. Histopathologically, psoriatic lesion is characterized by hyperplasia of keratinocyte, intraepidermal neutrophil accumulation and activated T lymphocyte infiltration in the dermis. Among many cytokines, IL-8 is known to play a role for the activation and migration for neutrophil and T lymphocyte and for the proliferation of keratinocyte. We performed immunohistochemical staining with anti-IL-8 monoclonal antibody for the active lesion, stable lesion and normal-looking skin of ten psoriatic patients and ten healthy controls. Strong expression of IL-8 in the epidermis and dermis was noted from the active as well as stable psoriatic skins. In the normal-looking skin of the psoriatic patients, only faint expression which is not different from healthy control skin, was noted. These observations indicate that the expression of IL-8 is increased in psoriatic lesion, and IL-S is important for the initiation and maintenance of psoriasis.

ABSTRACT

BACKGROUND: Job holders are groups that have suffered from chronic stressful condition. Nowadays, occupational stress are increasing due to Korean economic crisis. Therefore, this study was designed to evaluate the stress scores of several kinds of jobs, the relationship between the amount of stress and mental health status, factors associated with stress. METHODS: Among workers who had visited a medical college hospital, Taejeon, Korea, for regular health evaluation from April 1 to June 30 1998, we selected 50 clerks, 50 industrial workers, 50 professionals, 50 service or salesmen and 50 public servants in the survey. A total of 159 workers were evaluated as subjects excluding those who had answered in adequately to the questionnaire. Stress scores were measured by Korean-translated BEPSI(Brief Encounter Psychosocial Instrument) and Bae Jong Myun's 30-items SRRQ(Social Readjustment Rating Questionnaire). Mental health status were checked by Korean-translated SCL-90-R(Symptom checklist-90-Revision). RESULTS: The average BEPSI score of workers was 2.07+/-0.72. The mean score of female workers was 2.27+/-.70 and it was obviously higher than male worker's score of 1.85+/-.66. 20-29 year age group and over 40 year age group's amount of stress were obviously higher than 30-39 year age group. The low education level group had higher stress score than those with high education level but the amount of stress showed no significant difference in the monthly income, job tenure, number of family members to support, hobby and religion. According to occupation, clerks, industrial workers, service or salesmen's stress scores were higher than those of professional or public servants. CONCLUSIONS: This investigation has showed that there is a correlation between the amount of job holder's stress and mental health including somatimtian, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation and psychotism Therefore, we conclude that the best way to stay in good menta

ABSTRACT

Elastofibroma is a rare disease which presents as a slowly-growing, solid, ill-defined fibroelastic mass occuring almost exclusively in elderly women and arising mainly from the connective tissue between the lower part of the scapula and chest wall. Of the 270 cases of elastofibroma which have been reported, 170 cases had developed in Japanese patients. Despite the fact that there have been only two case reports in Korea, there might be a higher prevalence of elastofibroma in Korea than expected because of geographical similarity. We experienced a case of elastofibroma occurring in a 56-year-old man. To our knowledge, this is the first case report of elastofibroma in Korean dermatologic of literature.

ABSTRACT

Nerve sheath myxoma or neurothekeoma, a rare cutaneous neoplasm probably originating from the nerve sheath, is classified into three groups as classical, cellular, and mixed types based on cellularity, mucin content and growth patterns. Only two cases of nerve sheath myxoma have been reported in the Korean literatures. We report a case of nerve sheath myxoma occurring on the scalp of a 29 year-old woman. It was identified as the mixed type by histopathological, immunohistochemical and electronmicroscopic findings.

ABSTRACT

Ingrowing nail is that the soft tissue of the side of the nail is penetrated by the edge of the nail plate, resulting in pain, inflammation, the formation of granulation tissue. This disease is common in adolescence and adult, but rare in infant. So far, there has not been reported ingrowing nail associated with onychomycosis mycologically confirmed. We report a case of congenital hypertrophic lip of hallux, a subtype of infantile ingrowing nail, in a 6 month-old-male, associated with onychomycosis due to Candida parapsilosis.

ABSTRACT

Congenital laryngeal cleft is a rare anomaly, which can cause serious problems of airway and repeated aspiration pneumonia. It is due m failure of fusion of the posterior cricoid cartilage lamina. Dysphagia with aspiration of food is commonly seen w1h resultant bouts of pneumonia. Detection of an unsuspected minor cleft may be difficult, but the pediatrician should suspect the possibility of deft from the clinical features. This paper includes. a detailed case report of one patient with congenital laryngeal cleft and the review of literatures.

ABSTRACT

Clear cell sarcoma of the tendon and aponeurosis was first described by Enzinger in 1965, and has become a well-accepted clinicopathological entity as malignant melanoma of soft parts. Although it posesses the ability to produce melanin, it is quite different from conventional melanoma in that, it is more deeply located, usually associated with tendon and aponeurosis, displays pale-staining round to oval-shaped tumor cells and has a longer clinical course and better prognosis.Several cases have been reported in Korea, and we experiened an additional case of this rare tumor that showed ulceration.

ABSTRACT

BACKGROUND: The rhus has been known to be such a potent allergen that it causes severe contact dermatitis, and mary studies have tried to reveal the mechanism of allergic contact. dermatitis to urushiol. However, the mechanism of systemic contact dermatitis to urushiol is not, well-known, though the consumption of rhus as a health product and for desensitization is not unconmon in Korea. OBJECTIVE: The purpose of this study was to evaluate the niechanism of systernic contact dermatis, and we carried out irnmunohistochemical stainings un the two strands of dermatitis (systemic arid,llergic contact dermatitis). METHODS: The patients of systemir, contact dermatitis were divided into three groups by duration and severity of the illness according to the desired criteria. We perfor med immunohistor,hernical staining of the tissues from the patients with anti CD4, CD8, CDla, ICAM-1, IL-2R monoclorial antibodies. RESULTS: 1. The results were as follows. 1. The immunohistochemical staining showed that CD8 positive cell numbers in systemic contact dermatitis(18+/-10) were significantly lower than those of allergic contact der matitis(37 +/- 13) ( p0.05). CONCLUSION: The result suggest that lower numbers of CDH positive T cell in systemic contact dermatitis caused a reduation in the suppressor effect to the urushiol mediated immune rection and CD8 positive T cells play an important role in systemic contact dermatitis due to rhus.
